The rate per application is $450. Applications and payments must be received by August 15th, 2026.
Instructions
To apply for a new, first-time application, click on the green buttons below for the commendation for which you want to apply.
To renew a commendation, from the list below, find the site/paving division for which you want to apply next to the name of the appropriate commendation and click on "Renew" on the right-hand side.
If you’d like to Review and Print the questions before you begin here is a comprehensive list of the applications. Simply follow the link to download a PDF of the application before completing.
