The rate per application is $425. Applications and payments must be received by November 15, 2024. 

Instructions

To apply for a new, first-time application, click on the green button below for the commendation for which you want to apply. 

To renew a commendation, from the list below, find the site/paving division for which you want to apply next to the name of the appropriate commendation and click on "Renew" on the right-hand side.
