This is the portal for ordering all Diamond and Awards plaques, including Diamond Commendation, Larry H. Lemon, Sheldon G. Hayes, Ray Brown, Community Involvement, Asphalt Operations Safety Innovation, and Environmental Leadership Awards. 

To order a plaque:

1. Find the award for which you want to order the plaque, and the correct application name. 

2. Ensure that all information on the plaque form is correct. For Awards, information will pre-populate, but can be edited. You will not receive a proof for your plaque, so please ensure that all information is correct before submitting. 

3. Ensure that the shipping address that is entered is not a PO Box as UPS will not ship to those. 

4. Add the plaque order to cart and pay for your order.
